The learner will: Explain how claim rejections differ from denials in terms of processing and financial impact. Identify frequent payer edit rules, clearinghouse errors, and rejection codes encountered in claim submission. Analyze rejected claim data to determine the underlying cause and financial implications of the rejection. Apply corrective steps to address coding, data entry, or eligibility issues leading to claim rejections. Evaluate claim edit reports to identify staff training or process improvement needs. Create a rejection tracking log or tool to monitor trends, measure resolution timelines, and improve clean claim performance.
Before beginning this unit, learners should have: A solid understanding of claim submission and EDI transmission workflows. Familiarity with payer requirements and denial management fundamentals. Completion of Unit 4.2: Electronic Data Interchange (EDI) and ANSI Standards or equivalent claims processing experience.
