Description

This unit introduces learners to the principles and practice of Clinical Documentation Improvement (CDI)—a critical process for ensuring that patient records accurately reflect the care provided. Students explore how clear, complete, and compliant documentation impacts coding, quality reporting, and reimbursement. The unit emphasizes identifying documentation gaps, applying compliant physician queries, and improving interdisciplinary collaboration to enhance data accuracy. Learners will analyze sample notes, apply CDI templates, and design process improvements to support revenue integrity and clinical accuracy.

Explain Clinical Documentation Improvement (CDI) goals. Identify key documentation elements required for coding. Evaluate sample notes for compliance with CDI standards. Apply documentation templates for improved billing. Analyze errors leading to denials in documentation. Design a CDI improvement plan for a department.
The learner will: Explain the purpose and goals of Clinical Documentation Improvement and its impact on coding and reimbursement accuracy. Identify essential documentation components required to support coding specificity and compliance. Evaluate clinical documentation samples to determine adherence to CDI and regulatory standards. Apply structured documentation templates to enhance completeness and billing accuracy. Analyze documentation errors that contribute to claim denials and data discrepancies. Create a comprehensive CDI improvement plan addressing process gaps and departmental needs.
Before beginning this unit, learners should have: Basic knowledge of medical terminology, anatomy, and pathophysiology. Familiarity with ICD-10-CM, CPT, and HCPCS coding systems. Completion of Unit 3.1: Charge Capture Essentials or equivalent practical experience in healthcare billing or documentation.
