Learners will master coordinated care plan components including patient assessment, goal-setting, team roles, and monitoring mechanisms. They will understand diverse team member functions and how to integrate behavioral health and social services. Learners will apply SMART goals and RACI frameworks, use care planning tools and EHR systems, identify and address implementation barriers, design comprehensive plans for complex patients, and practice collaborative decision-making through simulations.
Basic familiarity with interprofessional teamwork, healthcare processes, and patient-centered care is helpful but not mandatory. Understanding of chronic disease management and care coordination fundamentals would enhance learning.
