This course introduces foundational principles of integrated care, an approach that prioritizes seamless coordination of healthcare services across sectors, disciplines, and settings. Learners explore key dimensions of integration—clinical, professional, organizational, and normative—and examine how integration supports value-based care and chronic disease management. The course uses international case studies and patient journey mapping tools to assess real-world applications and outcomes.

Learners will master the definition and purpose of integrated care including its role in reducing fragmentation and improving outcomes. They will understand core components such as multidisciplinary teams, shared information systems, and continuity of care. Learners will identify systemic and organizational barriers to integration, apply integration concepts to real cases, analyze interprofessional collaboration mechanisms, design patient-centered care plans, and use patient journey mapping to visualize system-level integration.

Basic familiarity with healthcare systems, patient-centered care principles, and team-based delivery models is helpful but not mandatory. Knowledge of chronic disease management and health policy concepts would enhance learning.
