Learners will identify systemic and organizational barriers to integration, apply integration concepts to real cases, analyze interprofessional collaboration mechanisms, design patient-centered care plans, and use patient journey mapping to visualize system-level integration.

Basic familiarity with healthcare systems, patient-centered care principles, and team-based delivery models is helpful but not mandatory. Knowledge of chronic disease management and health policy concepts would enhance learning.
