Description

Upon successful completion, learners receive a Certificate of Completion in Collaborative Care Models, demonstrating competency in implementing team-based care approaches and coordinating integrated care delivery.

Learners will master the core components of collaborative care models including multidisciplinary teamwork, patient-centeredness, care coordination, and evidence-based practice. They will understand the distinct roles of physicians, nurses, pharmacists, social workers, behavioral health specialists, allied health professionals, and patients within integrated care teams. Learners will apply collaborative practices to patient scenarios, analyze care model impacts on outcomes and system efficiency, evaluate collaborative care effectiveness in chronic disease management, design interdisciplinary care plans, and demonstrate effective teamwork through simulated collaborative care settings.
Basic familiarity with primary care structures, team-based care models, and healthcare system fundamentals is helpful but not mandatory. Knowledge of patient-centered care principles and chronic disease management concepts would enhance learning.
Healthcare professionals including physicians, nurses, pharmacists, social workers, behavioral health specialists, and allied health professionals engaged in collaborative and integrated care delivery. Particularly valuable for care coordinators, team leaders, and clinical managers working in primary care clinics, federally qualified health centers, and integrated care organizations who need to enhance care coordination and team-based service delivery.
