Learners will master the definition and principles of integrated case management including patient-centeredness and biopsychosocial approaches. They will identify and apply comprehensive assessment tools, understand diverse professional roles in teams, and develop individualized service plans. Learners will analyze care coordination gaps using root cause analysis and other methods, design comprehensive plans addressing medical and social determinants, and practice interprofessional collaboration through simulations with structured communication and shared decision-making.
Basic familiarity with healthcare delivery systems, chronic disease management, and interprofessional teamwork is helpful but not mandatory. Understanding of social determinants of health and care coordination principles would enhance learning.
