Learners will understand care transition components including discharge planning, medication reconciliation, and continuity of care. They will recognize team member roles in safe handoffs and their interdisciplinary responsibilities. Learners will identify discharge planning tools, apply SBAR communication effectively, analyze readmission root causes, design comprehensive transition plans addressing medical and social needs, and practice structured communication in simulated handoffs.
Basic knowledge of healthcare delivery systems, patient safety principles, and interprofessional communication is helpful but not mandatory. Understanding of discharge processes and care coordination concepts would enhance learning.
