Learners will master care pathway components and understand their role in standardizing care and reducing variation. They will understand navigator functions in removing barriers and supporting patient engagement. Learners will identify tracking tools and registries, apply pathway principles to clinical contexts, recognize patient barriers, design navigation plans addressing clinical and social needs, and practice referral tracking in simulations.

Basic familiarity with healthcare delivery systems, chronic disease management, and patient-centered care principles is helpful but not mandatory. Understanding of care coordination concepts would enhance learning.
