Learners will identify tracking tools and registries, apply pathway principles to clinical contexts, recognize patient barriers, design navigation plans addressing clinical and social needs, and practice referral tracking in simulations.

Basic familiarity with healthcare delivery systems, chronic disease management, and patient-centered care principles is helpful but not mandatory. Understanding of care coordination concepts would enhance learning.
