This course introduces essential digital tools and technologies that enhance care coordination across the continuum of care. Learners explore core functionalities of Electronic Health Records (EHRs), referral tracking systems, patient registries, and care management dashboards. The course emphasizes how tools like secure messaging, telehealth platforms, and population health analytics reduce fragmentation and support shared care planning. Practical exercises include EHR simulations and mock referral workflows to reinforce application skills.
Define key digital tools used in care coordination Describe how health information systems support team-based care Identify appropriate technologies for communication and documentation Apply coordination tools to a clinical workflow scenario Analyze how digital platforms enhance or hinder team performance Develop a tech-based care coordination workflow for a case study Demonstrate the use of secure messaging and shared platforms in simulation
Learners will identify and describe key digital tools including EHRs, referral systems, and dashboards. They will understand how health information systems enable team collaboration and data sharing. Learners will select appropriate communication technologies, apply tools to real workflows, critically evaluate platform effectiveness, design technology-enabled workflows, and practice secure messaging and shared platform use in simulated environments.
Basic computer literacy and familiarity with healthcare documentation is helpful but not mandatory. Understanding of electronic health records and digital health concepts would enhance learning.
